Proof of Identity.

One of the following documents displaying your picture and date of birth:

a) US or foreign passport. 
b) US Driver's License
c) US State ID. 
d) US Permanent Resident Card
e) Consular Identification (CID)

Proof of residency.

One of the following documents containing the applicant's name and residential address within the City and dated within the last 30 days:

a) Utility bill. 
b) Local property tax statement
